For your first appointment, we ask that you print the following three forms (below), complete them, then bring them with you to your first appointment:

  1. Practice Policies - This 1-page form explains our office policies, including professional service, hours of operations, prescriptions, test results, professional fees, no show policy, and cancellation policy. [CLICK HERE]

  2. Authorization Acknowledgment - This 1-page form explains your rights to privacy regarding your protected health information under the Health Insurance Portability & Accountability Act of 1998. [CLICK HERE]

  3. Patient History - This 6-page American College of Rheumatology form helps us get to know your current and past health. The more information you can complete, the better we can serve you. Please feel free to print this form double-sided to save paper. [CLICK HERE]

We understand being diagnosed with a rheumatic disease may be scary and confusing, but with the right knowledge and qualified rheumatologist and healthcare team, living well with rheumatic disease is quite possible.

MEDICATION REFILL POLICY

Please plan ahead for medication refills. To obtain a refill, call your pharmacy and request the refill (at least 48-72 hours) ahead of when you need your medication. The pharmacy will then contact WNY Rheumatology to authorize the refill. These requests will be processed during normal business hours (Monday-Friday). Please note that routine follow-up appointments with WNY Rheumatology and blood work are required to obtain medication refills.


CALLBACK PROCEDURE

Please allow 1-2 business days for WNY Rheumatology to return any non-urgent telephone calls. Lab results and x-ray results will be discussed at follow-up appointments unless there is an abnormality, in which case you will receive a call from the provider or nurse.


INSURANCE/REFERRALS

WNY Rheumatology welcomes new patients and accepts most insurances. Please inform us when your insurance changes.
If your insurance requires a referral from your Primary Care Physician, be advised that each patient is responsible to obtain this referral before their scheduled appointment with WNY Rheumatology, or we may need to reschedule the appointment. If a referral has expired, contact your Primary Care Physician for a new renewal before your next appointment with WNY Rheumatology.
